Key Responsibilities:

• *Interpret and verify orders in the pharmacy computer system with precision.
• *Compound and dispense pharmaceuticals while ensuring accuracy and safety.
• *Adhere to facility policies, procedures, and regulatory standards.
• *Monitor drug therapy regimens for contraindications, interactions, allergies, and proper dosing.
• *Provide clinical consultation to healthcare practitioners and suggest cost-effective therapeutic alternatives.
• *Maintain the hospital formulary, minimizing non-formulary procurements and promoting rational drug therapy.
• *Participate in multidisciplinary and bedside rounds to optimize patient care.
• *Conduct and analyze drug utilization reviews, creating actionable plans to encourage rational therapy.
• *Develop and mentor pharmacy clinical programs, including:
  • Drug consults, nutrition support, pharmacokinetics, and anticoagulant monitoring.
  • Pain management, IV to PO transitions, and renal dosing.
• Oversee and lead the Antimicrobial Management Program (AMP).

Qualifications:

• Must-Haves:
• *B.S. in Pharmacy or Pharm.D.
• *Minimum 1 year as a Pharmacy Clinical/Operations Specialist.
• *Minimum 5 years of hospital pharmacy experience in the area of focus.
• *Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
• *Active Pharmacy Consultant license (or state equivalent).
• Nice-to-Haves:
• *Completion of PGY2 residency or a pharmacy specialty fellowship.
• *Specialty certification, such as BCPS (Board Certified Pharmacotherapy Specialist).
• *Additional certifications, such as FCCP (Fellow of the American College of Clinical Pharmacy) or FASHP (Fellow of the American Society of Health-System Pharmacists).
• *Hospital pharmacy management experience.
• *ACLS certification.

Compensation and Benefits:

• Salary Range: $140,000 - $166,000 annually.
